Data as of November 1, 2026

Partial — state directory temporarily unavailable, data as of November 1, 2026. The New Mexico Health Care Authority’s provider search was offline when this page was built, so the assisted living list below holds only the 65 metro facilities we could read from HCA’s directory (209 listings statewide). It is not the complete list. Check HCA’s facility search for any facility not shown, or ask an advisor.

How are the area pages built?

Each area page is generated from the New Mexico Health Care Authority's assisted living directory and the CMS Provider Data Catalog: licensed assisted living facilities with their license number and beds, the nearest facilities and nursing homes with straight-line distances, hospice and home health offices, and the county's Area Agency on Aging.

Areas follow the ZIP code on each record, so a facility with an Albuquerque mailing address may sit in unincorporated Bernalillo County.

Which county agencies serve the Albuquerque metro?

Bernalillo County is served by the City of Albuquerque/Bernalillo County Area Agency on Aging; Sandoval, Valencia and Torrance counties by the Non-Metro Area Agency on Aging; and statewide programs such as the ombudsman, Adult Protective Services and the ADRC cover all four.

Bernalillo · Sandoval · Valencia · Torrance

Why do some areas cover several ZIP codes?

ZIP codes, city limits and community names do not line up neatly around Albuquerque: the South Valley is unincorporated but uses Albuquerque addresses, Rio Rancho spans two ZIPs, and the East Mountains cover three, so each area page lists exactly which ZIPs it includes.

If you know only a street address, find its ZIP on the facility row in the directory and follow it to the matching area page.
